Choosing to use Insurance or Not
A “qualified” diagnosis must be documented on the insurance claim in order to submit for insurance reimbursement. Choosing to bill for counseling sessions through your insurance carrier is an important decision you must make. According to federal regulations, you may choose to pay out-of-pocket and NOT bill through your insurance policy. Clients who so opt are called, “Private Pay clients”. Should this be your preference, we (Mind At Ease, LLC) would NOT have the authorization to share your records (including diagnosis) with your insurance company. The decision you make at the outset of services may be reversed at any time by completing a new form and updating your file. Our approach at Mind At Ease, LLC is to treat the issue(s) at hand, and we do not like to label when it is not necessary.

However, we understand that many choose to use their health insurance and therefore we accept Blue Cross Blue Shield PPO of Illinois.

If using insurance, make sure you ask these questions to your insurance provider prior to coming in for your appointment:

Do I have mental health benefits?
What is my deductible and has it been met?
What is my co-pay at the time of my visit?
How many sessions per calendar year does my plan cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?

Fee Structure:

Initial Individual Session (75-90 minutes): $175
Subsequent Individual Session (60+ minutes): $150
Subsequent Individual Session (45 minutes): $125
Missed Appointments or Cancellation with less than 24 hours notice: $60
